Screamin Jay Hawkins dies following surgery 14 February 2000 Cleveland native Screamin Jay Hawkins (Jalacy J. Hawkins) died on Saturday in a hospital near Paris, from complications following gastrointestinal surgery, at age 70. The ghoul fixated, wacko R&B legend, said to be a major influence for the likes of Ozzy Osbourne and Alice Cooper, may be best known for his 1956 track I Put A Spell On You (covered in 1968 by Creedence Clearwater Revival on their debut).
